Binge antecedents in bulimic syndromes: An examination of dissociation and negative affect.

Method: Using handheld computers, 33 women with bulimic syndromes provided ongoing self-reports on eating behaviors, affects, and dissociative experiences over 7- to 29-day intervals. Results: Multilevel modeling analyses indicated that negative affect and dissociation were elevated prior to binge episodes. In addition, antecedent effects of dissociation on bingeing were independent of those attributable to negative affect. Conclusion: Our results are consistent with models of binge eating that assign a causal role to negative mood and altered self-awareness. As such, our findings imply that binge eating is a multiply determined behavior linked to diverse cognitive and affective processes. © 2007 by Wiley Periodicals, Inc. Is There a Link between Traumatic Experiences and Self-Injurious Behaviors in Eating-Disordered Patients?

Eating Disorders, Jul-Sep2007, Vol. 15 Issue 4, p305-315, 11p, 4 charts Abstract: To find out more about the relationship between the presence of self-injurious behavior (SIB) and a history of traumatic experiences, we studied this link in 70 patients with an eating disorder (ED). The sample showed a high frequency of SIB (38.6%), particularly in patients with bulimia nervosa. We also found high percentages of self-reported experiences of physical (32.3%) and sexual abuse (47.7%). The presence of SIB turned out to be associated with a history of physical and/or sexual abuse. Patients who had suffered interpersonal abuse before the age of 15 were more likely to develop self-destructive behaviors. In line with other investigations, we found that high levels of dissociation and self-criticism differentiated sexually abused ED patients with SIB from those without SIB. Dissociative disorders: pathways to diagnosis, clinician attitudes and their impact.

Australian & New Zealand Journal of Psychiatry, Oct2005, Vol. 39 Issue 10, p940-946, 7p Abstract: Objectives: To study the attitudes and experiences of Australian clinicians with dissociative disorders and the paths to diagnosis and experiences of patients. Method: The attitudes of Australian clinicians to dissociative disorders and the experiences of patients were assessed by questionnaires. The clinicians were mental health specialists and a small number of general medical practitioners. The patients had all been diagnosed with a dissociative disorder. Results: Of the 250 clinicians, 21% reported experience with more than six cases on average of any one of the dissociative disorders, 38% with less than six, 42% with none; 55% regarded them as valid diagnoses, 35% dubiously valid and 10% invalid. Of the 55 patients, 76% reported delays in diagnosis (57%, >3 years and 25%, >10 years) with adverse consequences in 64%; 80% had experienced sceptical or antagonistic attitudes from clinicians, rated as destructive by 48%. They were disabled (60% rated as >50% impaired) and were heavy consumers of health services (48% hospitalized, 68% >5 times). There was considerable comorbidity including moderate or severe depression (96%), self-harm (68%), suicide attempts (69%), panic disorder (53%), eating disorders (75%), substance abuse (25%), poor physical health (44%), major interpersonal (70%) and sexual problems (90%). Patients rated individual psychotherapy as the most helpful treatment (90%) but medications, such as antidepressants, were also valued (60%). Conclusions: Although over half of the responding Australian clinicians thought that dissociative disorders were valid, the rest were dubious about their validity with 10% believing them to be invalid. Only 21% had considerable experience with the disorders. These findings may relate to some of the difficulties perceived by patients, which included delays in diagnosis, suboptimal treatment and negative experiences with clinicians. State dissociation in bulimic Eating Disorders: An experimental study.

International Journal of Eating Disorders, Jul2005, Vol. 38 Issue 1, p37-41, 5p Abstract: Background Dissociation and binge eating appear to have equivalent functions, lowering awareness of generalized threat and negative self-esteem. Dissociation can be a general characteristic of the individual, present in all situations (i.e., a trait), or it can be a time-limited reaction to a specific situation (i.e., a state). Previous studies have shown that the subliminal presentation of threat cues leads to increased bulimic behaviors. This experimental study examines whether dissociation is activated under similar conditions, thus testing whether dissociation is a reactive state in bulimic women, as well as a trait. Method The participants were 24 bulimic and 26 nonclinical women, each of whom completed the Eating Disorders Inventory and the Dissociative Experiences Scale. Each woman was exposed to subliminal neutral and threat cues, before completing the subjective component of the Clinician-Administered Dissociation Scale and a measure of anxiety and mood (the Hospital Anxiety and Depression Scale). Results The subliminal threat cue significantly increased state dissociation (particularly derealization levels) in the bulimic women but had no effect on the nonclinical group. There was no impact on mood. Discussion State dissociation appears to play an integral role in the bulimic process, but further studies are needed to determine the causal structure in that relationship and whether it applies in other disorders. Future clinical practice should be directed towards the identification and treatment of state dissociation in therapy. © 2005 by Wiley Periodicals, Inc. Characteristics of self-cutters among male inmates: Association with bulimia and dissociation.

The subjects were 796 male inmates of a juvenile prison. A self-reporting questionnaire was used to assess self-cutting, histories of psychoactive substance use, problem behaviors, and traumatic life events in the subjects. The Adolescent Dissociative Experience Scale and the Bulimia Investigatory Test of Edinburgh were also used. Subjects were divided into two groups: self-cutting and non-cutting. Questionnaire responses and dissociation and bulimia assessments were compared between the groups. Self-cutters began smoking (P < 0.001) and drinking (P < 0.001) earlier, and more frequently used illicit psychoactive drugs (P < 0.001), experienced childhood physical abuse (P < 0.001), and reported suicide attempts (P < 0.001), suicidal ideation (P < 0.001), and outward violence toward a person (P < 0.001) or object (P < 0.001) than non-cutters. Self-cutters also scored significantly higher on the bulimia (P < 0.001) and dissociation tests (P < 0.001). Logistic regression analysis demonstrated that suicide attempt (odds ratio, 4.311) and suicidal ideation (odds ratio, 2.336) could discriminate between male inmates with and without self-cutting. Male self-cutters showed‘multi-impulsive bulimic’ tendencies resembling those of female self-cutters, although to a lesser extent. Eating-disordered patients with and without self-injurious behaviours: a comparison of psychopathological features.

European Eating Disorders Review, Sep/Oct2003, Vol. 11 Issue 5, p379, 18p Abstract: A considerable number of eating-disordered patients also display all kinds of self-injurious behaviours (SIB), which might be viewed as an indicator of psychopathological severity. To test this hypothesis in 70 females admitted to a specialized treatment programme for eating disorders, a wide spectrum of psychopathological features was studied by means of self-reporting questionnaires: clinical symptomatology, personality disorders, aggression regulation, trauma history, dissociation and body experience. A comparison was made between patients with (n = 27) and without SIB (n = 43), as well as between patients with one (n = 13) versus more types (n = 14) of SIB. In general, patients with SIB reported significantly more complaints or signs of anxiety, depression, hostility, cluster B personality disorders, feelings of anger, traumatic experiences, dissociation and negative appreciation of body size. Relationship of dissociative experiences to body shape concerns in eating disorders.

European Eating Disorders Review, Jan/Feb2003, Vol. 11 Issue 1, p38-45, 8p, 3 charts Abstract: Objective: To examine the relationship of dissociative experiences to body shape concerns in a Spanish sample of eating disorders as compared to a general population. Method: 118 eating disordered outpatients and 64 controls completed the Body Shape Questionnaire (BSQ), the Rosenberg Self-esteem Scale (RSE) and the Dissociative Experiences Scale (DES). Scores were compared within different diagnostic categories. Regression analysis was used to assess the predictive power of the DES and RSES for the BSQ. Results: 36 (30.5 per cent) patients were found to be at high risk for dissociative disorders. In patients, DES together with RSES scores were predictors for BSQ. Discussion: These findings are consistent with the hypothesis that, in a subgroup of eating disorders, dissociation might be associated to the degree of dissatisfaction towards their own body and it might represent a way of coping with a negative self-image.
